Pick two dates and it returns the number of days between them, the same span as weeks and leftover days, the hours, and a years-months-days breakdown. Each date can be in BS or AD independently, which is the usual case when one date is on a Nepali document and the other on a foreign one. The second card goes the other way: start from a date and add or subtract a number of days to find the date that lands on.
BS and AD label the same days differently; they do not have a different number of days between two moments. Asar 15 to Bhadra 15 and the matching two AD dates are the same number of days apart. Only the months-and-days breakdown depends on which calendar's month lengths you count by, and this page counts it on the AD dates so the months have fixed, known lengths.
By default the count is the number of days between the two dates: 1 Baisakh to 2 Baisakh is one day. Leave, contract and visa periods usually count both the first and last day, so 1 to 10 Baisakh is ten days of leave. Tick "Count the end date too" for that convention; it adds one day to the total.
Notice periods and leave balances, visa and permit stays, loan and fixed-deposit terms, the age gap between two people (enter both dates of birth), days left until an exam or a wedding date, and deadlines stated as "within 35 days of publication" — for that last one, use the add-days card on the publication date.
Adding days by hand in BS goes wrong because you have to know how many days each month in between has that year — Shrawan might be 31 or 32. The add-days card converts the start date to AD, moves by the number of days, and converts the result back with the per-year BS tables, so month lengths are always the real ones.

Not by default — the count is the days between the dates. Tick "Count the end date too" to include both the first and last day, which is how leave and visa periods are usually counted.
Yes. Each picker has its own BS/AD switch, so you can count from a BS date on a citizenship certificate to an AD date on a visa.
Use the "Add or subtract days" card: pick the start date in BS, choose Add, enter 90 and calculate. The resulting date is shown in both BS and AD.
